Ingredient explainer

Semaglutide

Semaglutide is the active ingredient behind several brands. As a weekly injection it's Ozempic (registered for diabetes) and Wegovy (registered for weight management); as a daily tablet it's Rybelsus (registered in SA but not yet sold). It's one of the two most effective weight-loss molecules available.

What semaglutide is

Semaglutide is a GLP-1 receptor agonist: it imitates the gut hormone GLP-1 to reduce appetite, slow how fast the stomach empties and improve blood-sugar control. It was developed by Novo Nordisk for type 2 diabetes, and its powerful effect on appetite made it a landmark weight-loss medicine. Learn the biology in how GLP-1 works.

Brands & forms in South Africa

  • Ozempic — weekly injection, registered for type 2 diabetes; used off-label for weight loss.
  • Wegovy — weekly injection at higher doses, registered for chronic weight management (launched in SA in 2025).
  • Rybelsus — a once-daily oral semaglutide tablet, registered in SA but not yet marketed here as of 2026.

The injectable brands differ mainly in registered use and maximum dose — see Ozempic vs Wegovy.

How well semaglutide works

In the STEP 1 trial, semaglutide 2.4 mg produced about 15% average weight loss over 68 weeks in adults without diabetes. It's second only to tirzepatide on average weight loss — see semaglutide vs tirzepatide. Results vary between individuals.

“Compounded” & “generic” semaglutide

You may see cheaper “compounded” or “generic” semaglutide advertised. These are not SAHPRA-registered products and carry real safety and quality questions. Read our honest, balanced explainer first: compounded semaglutide in South Africa.

Avoid counterfeits

SAHPRA has warned about falsified semaglutide circulating in South Africa. Only use products dispensed by a licensed pharmacy on a valid prescription, and be wary of anything sold on social media, by couriers without a script, or at prices that look too good. See our compounded & counterfeit guide.
